Medical Evaluation Requirements

Services are provided only after a required intake and assessment. If you have emergency symptoms, seek immediate care (call 9-1-1or go to the nearest ER). Any diagnosis/medical decision will be based on the information you provide and any required follow-up evaluation. You may be asked for records, vitals, or additional screening if needed for safety.

Patient Responsibilities

Provide accurate, complete medical history and current medications. Disclose known allergies, chronic conditions, and any relevant test results. Use the service as directed and follow safety instructions for medications or treatment plans. Report worsening symptoms promptly and request reassessment when required.

Refund/Cancellation Policies

Cancellations must follow the stated time window for the specific service/visit type. Refund eligibility depends on whether services were already performed and the status of your appointment or order. Nonrefundable fees may apply for scheduled appointments, processing, or reserved time. Refunds, if approved, are issued to the original payment method within a reasonable timeframe.

Jurisdictional Limitations

This agreement and services are governed by the laws of the State of Arizona. Any disputes must be handled in the appropriate courts located in Arizona. We make no guarantee that services comply with laws outside Arizona jurisdictions. If any provision is found unenforceable, the remaining terms remain in effect.
